About Swan View

Swan View is transitional accommodation providing safe, secure, comprehensive, culturally appropriate and flexible supports for up to 12 months. Support is based upon the individual needs of people with a mental illness with input from their families and/or carers. The service will support participants to transition to the NDIS and find suitable long term sustainable housing and supports.

About the role

As a Mental Health Support Worker you will work directly with consumers to provide supports that are client centred, safe, and aimed at achieving positive outcomes, whilst promoting independence, choice and decision making. You will support and maintain consumer welfare and interests at all times and models professional behaviours including a high degree of objectivity, integrity, and compassion.

Morning Shift 07:30 - 15:30

Afternoon Shift 14:30 - 22:30

Night Shift 21:30 - 07:30

Contracts will be for 5-10 shifts per fortnight, Monday to Sunday. Contact end date of the 30th June 2022 with a possibility of extension.

Key responsibilities:

  • Support consumers to develop their own support plans in partnership with Life Without Barriers, Community Mental Health services and carers
  • Monitor and review consumers’ progress through the service and against the case plan and provide up to date progress reports to the partner organisations
  • Increase Clients’ skills, independence, personal growth, and development through the provision of active support and their enhanced participation as a valued member of their community. This includes providing individualised support such as:
    • Establishing routines and structures
    • Assisting in establishing and maintaining stable and appropriate accommodation where this does not exist
    • Coaching and guiding consumers in day-to-day life skills such as cleaning, cooking, shopping, managing finances etc
    • Promoting, encouraging, and ensuring positive relationships with parents, family members, and other persons significant to the client
    • Identifying programs, encouraging, and advocating consumers involvement in educational, vocational, and recreational programs that support practical living skills and community reintegration
  • Assist the delivery of programs that facilitate the clients’ development of social networks and inclusion in community activities
  • Engage and develop rapport with consumers who are experiencing mental illness and other complex needs to understand their needs and to help them identify problems and areas for improvement and implement effective solutions
  • Provide an appropriate model to consumers in terms of dress, language, and behaviours
  • Monitor and maintain high quality documentation relating to care plans, progress notes and other documentation in line with funding body and licensing requirements
  • Use technology as required to complete documentation, access client files and other resources

Skills & Experience

  • Certificate IV in Mental Health or similar
  • Demonstrated ability to positively engage, interact and work with people who have complex needs including a psychiatric disability.
  • Sensitivity to work with people from a diverse range of cultural backgrounds, particularly people from Aboriginal and Torres Strait Islander backgrounds, and a willingness to learn and work in culturally appropriate approaches.
  • Computer literacy and administration skills
  • Current driver’s license
  • Current Provide First Aid & CPR certificate 

Successful candidates will be required to clear probity checks including National Criminal History Record Check and NDIS Worker Screening check.
