About the Role

You will actively support older people to enable opportunities for them to become more socially connected into the local community. Building on existing relationships, you will support strengthen personal and community connectedness and reduce social isolation. Working up to 15 hours per week Monday - Friday, you will also support older people to become more aware of healthy lifestyle options and develop targeted activities to meet individual and group needs. This is a current max term contract to the 31st July 2023.

Key Responsibilities

  • Provide direct quality support to clients in line with their needs and goals
  • Promote empowerment approaches
  • Increase the client's skills, independence, personal group and development through increased participation in their community 
  • Assist in the delivery of services that facilitate client’s development of social networks and inclusion / assistance with community activities in line with LWB Policies and Procedures.
  • access existing community services and resources where possible as well as develop targeted activities to meet individual and group needs

  Skills & Experience

  • A current unrestricted drivers licence and a reliable vehicle with minimum 3rd party insurance
  • A current First Aid Certificate
  • Certificate III in Aged Care, Home and Community Care, Disability or similar
  • Prior experience supporting working in the community with ageing clients is highly regarded
  • The ability to develop positive relationships with the clients and to support their individual care plans as appropriate

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
