About the opportunity
We currently have an exciting opportunity for a Case Manager (Support Advisor) to join our friendly and supportive Sydney Central Home & Community Care team based in Lilyfield for a 2 year fixed term contract. We are looking for someone to manage a specialised caseload, working with people who are experiencing or are at risk of homelessness across the Sydney Central region.
Key responsibilities include:
- Advocating and managing care for older people who are experiencing or at risk of homelessness
- Providing services tailored to the needs of each individual client
- Providing person-centred assessment and case management
- Providing cost effective, evidence based and goal orientated case management while working in partnership with clients to achieve positive outcomes
- To build strong links with the community and housing services aspects of the aged care industry
About you:
- Relevant/equivalent qualifications and experience in aged care, allied health, nursing or mental health
- Knowledge and understanding of NSW Residential Tenancies Act 2010, programs and funding available to older Australians including Commonwealth Home Support Program (CHSP), Assistance Care and Housing (ACH), Home Care Packages (HCP) and Social and Affordable Housing Fund (SAHF)
- Knowledge and skills in working with people who are socially isolated, experiencing or are at risk of homelessness, hoarding and squalor
- Ability to build rapport and trust with clients and colleagues
- Strong skills in advocating for clients with a range of external stakeholders
- Excellent verbal and written communication skills
- Proficient in using computer applications including Microsoft office, client/data management systems and a willingness to learn new programs
- Current First Aid or willingness to obtain
- Australian Drivers License and access to your own vehicle
