About the Role

As a member of our community-based team, you will facilitate the delivery of positive and proactive support for people with disability. You will actively support individual needs and create a safe environment for them to reach their goals. The successful candidates will have experience with intensive positive emotional support and have worked with people with a range of physical, intellectual and psychosocial disabilities.

The role will be providing support in the person's home including; personal care, medication administration and household tasks.

We provide community support Monday to Sunday (daytime), and you must be available for a minimum of 3 days a week (this can include weekends).

We have shifts available in Midland, Belmont and Canning Vale. Shifts are Monday/Tuesdays and between the hours of 9:00 and 14:00.

Key Responsibilities

  • Assistance with daily living (including personal care where needed)
  • Community access and transport (with the use of your own vehicle)
  • In-home care includes assisting with basic household tasks 
  • Medication administration
  • Maintain and monitor person-centred plans
  • Documenting and progress note keeping

Skills & Experience

  • Experience supporting people with disability (essential)
  • Australian Drivers Licence and reliable vehicle (essential)
  • Current Provide First Aid Certificate or willingness to obtain prior to commencement (essential)
  • Willing to complete basic household chores or assist the person you support to complete the tasks 
  • Willingness to deliver personal care - assisted showering, dressing, brushing teeth etc.
  • Passionate about delivering Person-Centred care

Successful candidates will be required to clear probity checks including a National Criminal History Record Check and obtain an NDIS Clearance.
