About the role

As a Case Manager within our Family Based Care team located in Newcastle, your role will involve close collaboration with children, young individuals, carers, and families to meticulously formulate and execute individualized case plans. These plans are designed to actively advance and champion the permanency objectives for the children and young individuals under our care

To thrive in this highly rewarding opportunity, you'll need a background in working with Children & Young People in out-of-home care, coupled with expertise in managing complex caseloads.

This is a Permanent Full Time position offering a blend of on-site and remote work arrangements - Regular travel to the locations of the Children, Young People, Families and Carers is a requirement of this role, and a company vehicle is provided for this.

Key Responsibilities 

  • Develop and implement high quality case plans or individual plans for children and young people in care.
  • Develop and maintain functional and supportive relationships with children and young people in family-based care as well as their families to reach positive outcomes.
  • Support and monitor placements and ensure carers participate in the case planning process and assist them to implement the strategies outlined in the case plan.
  • Meet with clients and family members to gather input for case plans and ensure their views are represented.
  • Ensure compliance with all internal policy and procedure, statutory and contractual requirements in relation to management of service delivery.

 Skills & Experience

  • Relevant bachelor’s degree in social work, Social Welfare, Psychology, Nursing or Mental health (A qualification is desirable but not essential for Aboriginal and Torres Strait Islander staff with appropriate experience and willingness to undertake further training)
  • Demonstrated experience in case planning with children and young people
  • Experience in OOHC or child protection preferred
  • Working understanding of the NSW Permanency Support Program
  • Experience working autonomously in implementing therapeutic case plans for multiple clients concurrently
  • Current driver's licence
