As a Case Manager within our Family Based Care team in Wollongong & Southern NSW , you will collaborate closely with children, young people, their carers, and families to create and implement case plans that strive towards achieving the permanency goals of those we support.
In this dynamic role, You'll build and maintain strong, functional relationships with both the children and their families, ensuring that every voice is heard and every need is met. Your efforts will be pivotal in supporting and monitoring placements, guiding carers in their foster care journey and helping them implement effective strategies that best support the children and young people within their care.

Role requirements:
Successful candidates will be required to clear probity checks including National Criminal History Record Check, NSW Working with Children Check and NSW Residential Care Workers Register check.
