About the Role
The purpose of the Child and Family Practitioner Mentoring and Personalised Support Service (MAPSS) is to engage and support young people referred to the MAPSS program to improve outcomes across areas of safety, health and wellbeing, social and emotional development, education, and learning. The incumbent works in partnership with the young person and their care team to develop and implement high-quality, individually tailored Care Plans, outlining goals which draw upon the young persons strengths and addressing identified needs.
The objective of the position is to support young people to achieve higher outcomes across ecological domains which correspond with the Child and Youth Wellbeing framework, using the Outcomes Star tool to capture the voice of the young person and their needs, and to assess and measure progress across these domains.
This is a Full Time 12 Month Max Term appointment based in Hobart. A Tool of Trade vehicle (including fuel card) will also be allocated to the successful candidate.

Successful candidates will be required to clear probity checks including National Criminal History Record Check and hold Tasmanian Working With Vulnerable People Registration.
