About the Role
The purpose of the Case Manager Transition from Care (TFC) position is to provide outreach style case management support to young people between the ages of 15 and 18 who are transitioning from care to adulthood and independence.
The successful candidate will work in partnership with the young people, carers, relevant government agencies and stakeholders to develop skills and implement individualised care plans.
This position is being offered is full-time based at our office in Robina. A Tool of Trade vehicle (including fuel card) will also be allocated to the successful candidate.

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
