About the Role
As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Toowoomba and surrounding local areas. Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.
Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems. You will deliver exceptional service to ensure our clients and their families feel valued and supported, while also fostering growth among our aged care clients.
This role is available due to growth in our services in the Darling Downs region. This position is based in our Toowoomba office, with some travel required to clients' homes as needed across Darling Downs, from Clifton to Crows Nest and Oakey. This is a hybrid working opportunity where you will be able to work some days each week from your own home after your first few months of training in the office.
