About Local Area Coordination Partners in the Community (PITC) Program

The NDIS Partners in the Community Program for Local Area Coordination (LAC) Services works in partnership with the National Disability Insurance Agency (NDIA) to assist people with disability, their families and carers to achieve tangible outcomes, exercise choice and control and engage with the National Disability Insurance Schemes (NDIS).

Uniting LAC support people aged 7-65 who have NDIS plans and are eligible for the Scheme, as well as connect people, who are not eligible for the Scheme, to services and supports in their local community.

Uniting is delivering Local Area Coordination in the local communities of Hunter New England including the following LGAs:

Armidale Dumaresq, Cessnock, Dungog, Glen Innes Severn, Gloucester, Great Lakes, Greater Taree, Gunnedah, Guyra, Gwydir, Inverell, Lake Macquarie, Liverpool Plains, Maitland, Moree Plains, Muswellbrook, Narrabri, Newcastle, Port Stephens, Singleton, Tamworth Regional, Tenterfield, Upper Hunter Shire, Uralla, Walcha

Role Responsibilities:

  • Working with people with disability, their families and carers at all stages of the NDIS participant pathway alongside implementing the Information, Linkages and Capacity Building (ILC) Policy Framework for those outside the NDIS
  • Providing support to prospective NDIS participants to engage with the Scheme
  • Assisting NDIS participants to implement their plans, including building community inclusion and capacity building
  • Complete plan reviews and full scheme plans with NDIS participants
  • Connecting people, who are not eligible for the Scheme, to services and supports in their local community

Are you the person we are looking for?

You will be an active community member who is enthusiastic about supporting and empowering people with disability and their families. We are looking for people who have a valid driver’s license, a minimum of 12 months’ work experience in the community services sector and/or lived experience of disability.

Qualifications in the community services sector e.g. disability, allied health, social work or community development will be highly regarded.
