About the Role

Provide one on one, in-home support to people with disability living in various suburbs of the Northern Beaches, including Dee Why, Manly and surrounds.

You will be required to travel to different suburbs of Northern Beaches during your shift in order to provide in-home support to individual people. Permanent part time (34 hours per fortnight) or casual positions available.

In order to fulfil the permanent part time position you will be required to work a set roster which consist of split shifts throughout the week. Travel is required in between split shifts.

Week 1:

• Mon 7-8am and 8:15 – 11:15am
• Tue 7-8am and 8:15-9:15am
• Wed 7-9am; 4:30-5:30pm and 5:30 – 6:30pm
• Thu 7-8am; 8:15-11:15am and 11:45am – 12:45pm

Week 2:

• Mon 7-8am and 8:15 – 11:15am
• Tue 7 – 9am
• Wed 7:30 -8:30am; 8:45 – 11:45am; 12 – 1pm; 4:30 – 5:30pm and 5:30pm – 6:30pm
• Thu 7 -8am and 8:15- 11:15am
  
  

Key Responsibilities

• Assistance with daily living (including personal care where needed)
• Community access and transport (with the use of your own vehicle)
• Domestic assistance (support with household chores)
• Maintain and monitor person-centred plans
• Documenting and progress note keeping

Skills & Experience

• Experience supporting people with disability 
• Australian Drivers Licence and reliable, insured vehicle 
• Current First Aid and CPR
• Willing to complete basic household chores
• Confidence and experience in providing personal care
• Passionate about delivering Person-Centred care
