About the roles:

We are looking for 2 individuals to work in our part-time lines at our group home in Hampton. You will be responsible for providing support with activities of daily living and developing, implementing, and monitoring Individual Plans including personal care and implementation of complex care plan requirements as directed by health professionals. Clear and strong written communication skills are required to ensure reports and client notes are as detailed as possible. In this role you will work to facilitate the delivery of quality services and achieve positive outcomes with and for the clients within the house.

The roster details outlined as fortnightly hours is detailed below:

Roster Hours:

Roster splits – 67:00/59:30. This roster is a mix of mornings, afternoons, some split shifts and some full day shifts. Including 4 Sleep over shifts.

Roster splits - 59:30/53:00. This roster is a mix of mornings & afternoons, some split shifts and some full day shifts. Including 8 Sleep over shifts.

 

Key Responsibilities

  • Contribute to the day-to-day running of the house
  • Develop planned activities in line with policies and guidelines
  • Liaise with family members, medical services, departmental services, and external services to help meet client needs
  • Administer medication treatments and therapies according to prescribed protocols or otherwise, seek appropriate medical assistance

Essential Qualifications:

  • A current Senior First Aid (level 2) Certificate
  • A current unrestricted Driver Licence
  • A commitment to supporting and promoting the individual needs, skills, abilities and personal goals of people with disabilities
  • The ability to develop a positive relationship with service users and to support their Individual Personal Plan
